Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on a low-slope roof, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. At the jobsite located 9 West Allen Street, Wentzville, Missouri, on or about May 9, 2017, employee(s) for Daryl Behlmann Exteriors were engaged in roofing activities on a low-slope roof with unprotected sides and edges without any means of fall protection. The fall distance to the next lower level was approximately 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the jobsite located 9 West Allen Street, Wentzville, Missouri, on or about May 9, 2017, employee(s) for Daryl Behlmann were observed using aluminum portable extension ladders to access each level of the roof without extending the side rails at least 3 feet above the landing surface. The hazard was observed in the following instances: a. The aluminum portable extension ladder used to access the first level of the roof from the ground was extended approximately 18 inches above the upper landing surface; the fall distance to the ground below was approximately 9 feet. b. The portable extension ladder use to access the upper landing of the roof was extended approximately 18 inches above the upper landing surface; the fall distance to the next lower level was approximately 10 feet.
