Two employees were shingling a two-story motel with a roof pitch of approximately 5 on 12 without the use of fall protection. This condition exposed employees to fall hazards of approximately 20 feet.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    (a) Tin Man Roofing at 3155 Ingles Ln, Steamboat Springs, CO 80487: On or before 06/01/17, the employer did not insure that employees were properly using a portable ladder for access to the rooftop of a La Quinta. The employees were accessing the roof to perform roofing work, and the ladder did not extend at least 3 feet above the surface being accessed nor was the ladder secured to a rigid support. This condition exposed employees to fall hazards from approximately 20 feet.
