An in-patient hospitalization of one (1) employee, occurring on or about May 20, 2017, was not reported to OSHA within the 24-hour reporting requirement.    Wal-Mart was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1059076, citation number 2, item number 1, and was affirmed as a final order on August 31, 2015, with respect to a workplace located at 6626 FM 1960 Road, Humble, Texas 77346.    Wal-Mart was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1094997, citation number 1, item number 1, and was affirmed as a final order on February 12, 2016, with respect to a workplace located at 690 Old San Antonio Road, Buda, Texas 78610.    Wal-Mart was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1130856, citation number 1, item number 1, and was affirmed as a final order on April 7, 2016, with respect to a workplace located at #65 Wal-Mart Drive, Mountain Home, Arkansas 72653.    Wal-Mart was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1222526, citation number 1, item number 1, and was affirmed as a final order on June 20, 2017, with respect to a workplace located at 9700 N.
