29 CFR 1910.25(b)(4): Stairway landings and platforms are not at least the width of the stair and at least 30 inches (76 cm) in depth, as measured in the direction of travel:  (a) Entrance to the Digital Press Room: On or about June 8, 2017 and times prior, an employee uses a door that is at the top of a set of steps to enter and exit the digital press room. The door frame is approximately 17 inches from the edge of the first step.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stairway railings in accordance with Table D-2:  (a) Entrance to the Digital Press Room: On or about June 8, 2017 and times prior, an employee would access the copy room via stairs that were approximately 48 inches wide, had 5 treads and 6 risers, enclosed on one side and was not equipped with a stair rail system with a hand rail on the open side nor a hand rail on the enclosed side.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Facility: On or about March 1, 2017 and times prior, employees service equipment such as the folding and stitching machines without a program to address applying locks on equipment to prevent the startup or release of stored energy and did not address employee training.
