29 CFR 1910.1048(m)(2)(ii): The employer did not assure that for all materials listed in paragraph (m)(1)(i) and (iv) of this section, capable of releasing formaldehyde at levels of 0.1 ppm to 0.5 ppm, labels identified that the product contained formaldehyde; list the name and address of the responsible party; and state that physical and health hazard information was readily available from the employer and from safety data sheets:   (a) Eye Consultants of Pennsylvania; Blandon Facility - The employer did not label the medium-density fiberboard shelving units in the basement with information that the boards contained formaldehyde, the name and address of the manufacturer, and that health hazard information was available from the employer, on or about June 8, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1048(n)(2): The employer did not provide employees with information and training on formaldehyde at the time of their initial assignment and/or wherever a new hazard from formaldehyde was introduced into their work area:  (a) Eye Consultants of Pennsylvania; Blandon Facility - The employer did not provide employees with information and training on formaldehyde after the medium-density fiberboard shelving units were installed in the basement, on or about June 8, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
