29 CFR 1926.451(e)(1): An access ladder or equivalent safe access to scaffold(s) was not provided:  a)  On or about May 31, 2017, at the above addressed jobsite - an employee performing carpentry operations from a ladder jack scaffold did not have an additional ladder for safe means of access, thereby exposing the employee to fall hazards of approximately nine (8) feet six (6) inches.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a) On or about May 31, 2017 at the above addressed jobsite - employees performing framing/carpentry operations were exposed to fall hazards due to the employees working near unprotected sides and edges, thereby exposing the employees to fall hazards of approximately thirteen (13) feet and seven (7).
