29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about  June 8, 2017, there were no energy control procedures developed or utilized for multi energy source equipment. Employees perform such task as, maintenance, unjamming and tool/die changes. Employees exposed to the hazard of being caught-in the machine and/or an electrical shock hazard. Machines such as, but not limited to:     1. CNC machines called Cincom Citizen B-12s, there are 54 to 55 of these machines which have such energies as pneumatic and electrical.   2. Two secondary machines called Cincom Citizen Type B 12, No Z3281, these machines have such energies as pneumatic and electrical.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:  On or about June 8, 2017, the employer failed to ensure the employees were trained and they had the knowledge and skills for the safe application, usage and removal of energy control devices when maintaining, cleaning, unjamming, and  tool/die changes for multiple energy source equipment such as the Cincom Citizen B-12s and the Cincom secondary machines. Employee exposed to the hazard of being caught-in the machine or an electrical shock hazard.      1.CNC machines called Cincom Citizen B-12s, there are 54 to 55 of these machines which have such energies such as pneumatic and electrical.   2.
