29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from an aerial lift.    a) At Kohls Department Store located at 79 Weibel Ave, Saratoga, NY - On or about June 9, 2017, an employee was working in an aerial lift approximately 40 feet in the air with no fall protection exposing the employee to a fall hazard.
