29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about June 9, 2017, employees were exposed to struck by hazards.  The employees were performing roofing activities on a residential home located at 236 Churchill Hubbard, Girard, Ohio 44420.  Employees were working on all areas of the roof while other employees were on the ground without wearing protective helmets.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    On or about June 9, 2017, employees were exposed to a fall of approximately 9 feet.  The employees were performing roofing activities on a residential home located at 236 Churchill Hubbard, Girard, Ohio 44420.  The employees were working on all areas of the roof without using any form of fall protection.
