29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about June 9, 2017 at building 800, 725 Hwy 96, Bonaire, Georgia, five employees working on a low-slope roof two stories above the lower level, used an extension ladder to access the upper level. The side rails on the extension ladder did not extend 3 feet above the upper landing being accessed.
