29 CFR 1910.135(a)(2): The employer did not ensure that a protective helmet designed to reduce electrical shock hazard was worn by each affected employee when near exposed electrical conductors which could contact the head:  a) 114 Hitching Post Drive, Wilmington, DE - On or about June 12, 2017, the employer did not ensure that each employee with a potential for contact with overhead electrical lines were provided with and wore protective helmets designed to reduce electrical shock hazards.  An employee engaged in tree trimming activities within close proximity to overhead electrical lines experienced an electrical shock.

29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started:  a) 114 Hitching Post Drive, Wilmington, DE- On or about June 12, 2017, the employer failed to ensure that overhead lines, in close proximity to where work was to be performed, were deenergized and grounded, or other protective measures provided.  An employee engaged in tree trimming activities sustained an electric shock while working in close proximity to overhead lines.

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person, and the longest conductive object he or she could contact, could not come closer to any unguarded, energized overhead line, than 10 feet:   a) 114 Hitching Post Drive, Wilmington, DE- On or about June 12, 2017, the employer failed to ensure that employees engaged in tree trimming activities maintained a distance of 10 feet from energized overhead lines.
