29 CFR 1910.67(c)(2)(v): The employer did not ensure a personal fall arrest or travel restraint system that meets the requirements in subpart I was worn and attached to the boom or basket when working from an aerial lift: On or about June 9, 2017, an employee was exposed to a fall hazard of approximately 15 feet while in the bucket of a vehicle mounted aerial lift measuring and installing channel letters and the employees personal fall arrest or travel restraint system was not attached to the aerial lift.
