29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 6/7/17 at a roof replacement project located at 48 Apple Blossom Boulevard, Lancaster, New York; employees working on the roof were placing shingles on the roof of a two story house. The roof had a 6 in 12 pitch and an eave heights of approximately 20 feet. There was no fall protection in use. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 6/7/17 at a roof replacement project located at 48 Apple Blossom Boulevard, Lancaster, New York; an extension ladder used for access to the roof was only 1'-6" above the roof surface. NO ABATEMENT CERTIFICATION REQUIRED
