Jobsite: On or about June 14, 2017, the employer did not certify that each forklift operator was trained and evaluated as required by this paragraph (l).  The employer did not ensure an evaluation of the three powered industrial truck operators was conducted.  A written test was accomplished but the performance evaluation was not accomplished nor was a the required certification accomplished.
