Employees were observed climbing under a train that had been momentarily stopped to travel to the facility office.

29 CFR 1917.17(o): Warning signs were not posted where there was insufficient clearance for personnel between railcars and structures:  (a)  Facility, Buncher Industrial Park, Leetsdale, PA:  On or about June 13, 2017 there were no warning signs posted in the area of the shaker house where the railcars entered/exited the building.  There was insufficient clearance for personnel to pass between the building opening and railcar along the riverside and no warning signs were posted to inform personnel of the limited clearance.  Employees passed through this area to access the work area at the shakers and feeders.

29 CFR 1917.26(f): A US Coast Guard approved 30 inch life ring, with at least 90 feet of line attached, was not available at readily accessible points at each waterside work area where the employees' work exposed them to the hazard of drowning:  (a) Facility, Buncher Industrial Park, Leetsdale, PA:  On or about June 13, 2017 the employer has approximately 1/8 of a mile of waterfront area and only one life ring was available.  In addition, the life ring did not have at least 90 feet of line attached.  Employees pass along the waterside and others work on the barges and life rings were not readily available in the event of emergency.

Employees conduct maintenance on the conveyor system on a weekly basis that includes changing rollers, greasing and adjustment to the belt and the conveyor is not locked out when these tasks are performed.
