29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    (a) Facility, 4900 Grand Avenue, Pittsburgh, PA 15225: On or about June 15 2017, during maritime repair activities, Elements of a written program were not implemented to ensure that any employee using a respirator voluntarily was medically able to wear that respirator. Employees voluntarily wear a tight fitting half mask respirator when conducting welding operations.

29 CFR 1910.134(e)(6): 29 CFR 1910.134(e)(6):  In determining the employee's ability to use a respirator, the employer shall obtain a written recommendation regarding the employee's ability to use the respirator from the PLHCP (physician or other licensed health care professional):  a) Facility, 4900 Grand Avenue, Pittsburgh PA 15225, on or about June 15th, 2017: Employees, who voluntarily wear a respirator, are not provided a medial evaluation prior to donning a respirator.  Employees voluntarily wear a half mask air purifying respirator when conducting welding operations.
