29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  (a) Collimator Area - Roller Press was inadequately guarded, in that the installed plastic guard was adjusted to approximately 5/8 inch with the point of operation approximately 2-1/2 inches from the guard. On or about June 15, 2017 an employee operated the roller press to form lead sheets.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
