29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:      (a)   Jenesis Roofing at 9701 Dahlia Lane, Thornton, CO 80229:  On or before June 15, 2017, the exposing employer did not ensure that employees were protected from injury, in that three employees installing shingles on a two-story residential home, with a roof pitch of approximately 8-in-12,  were not utilizing a fall protection system.  The employees wore fall  harnesses; however, they were not connected to the fall protection system that was installed on the roof.  This condition exposed the employees to a fall of approximately 25 feet.    Jenesis Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1008798, Citation Number 1, Item Number 1 issued on December 19, 2014 and became Final Order on January 29, 2015, with respect to a workplace located at 1117 S. Grant St., Denver, CO 80210.
