29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about June 14, 2017, at the above addressed jobsite, an employee performing residential masonry operations twenty (20) feet two (2) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.    b.  On or about June 14, 2017, at the above addressed jobsite, an employee removing scaffold from PIV forks on a rooftop nineteen (19) feet six (6) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.    c.  On or about June 14, 2017, at the above addressed jobsite, employees performing residential masonry operations were exposed to an unprotected stairwell that was nine (9) feet from the basement floor, thereby exposing the employee to fall hazards.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  a.  On or about June 14, 2017, at the above address, an employee was required to operate a powered industrial vehicle and had not been provided with training, including but not limited to formal instruction, practical training and an evaluation of the operators performance, thereby exposing employees to struck by and crushing hazards.
