On or about June 16, 2017 at or near 8709 Stillwater Rd, Sherwood, AR 721120, the employer failed to provide safety glasses to employees operating nail guns during framing construction. This condition exposed employees to struck-by hazards from flying objects that could cause physical harm to their eyes.

Employees framing a single residential building worked on a platform with a single plank that was about 6 inches wide and had an approximately 20 inches opening to the wall.

On or about June 16, 2017 and there prior at or near 8709 Stillwater Rd, Sherwood, AR 721120, the employer did not ensure that the cantilevered portion of the single planked platform was at most 12 inches or had guardrails to block employees from accessing the cantilevered end. The cantilevered portion of the platform was more than 24 inches, creating a potential tipping and fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about June 16, 2017 at or near 8709 Stillwater Rd, Sherwood, AR 721120, employees were exposed to the following fall hazards:-     The employer exposed employees that were framing a rooftop about 13 ft 8 in to fall from height hazards. Employees were working on a 12/12 pitched roof without any type of fall protection.
