29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:   Location: Corian Sanding/Gluing Room  Employees were exposed to noise levels above the Action Level, 85 dBA, and the employer had not developed a Hearing Conservation Program, one that included initial noise monitoring, annual audiometric tests, hearing protection training, and the choice of a variety of hearing protection devices, as well as training on their use.

29 CFR 1910.1025(l)(1)(i): Employee(s) working in an area where there is potential exposure to airborne lead at any level were not informed of the content of Appendices A and B of 29 CFR 1910.1025.  Employees were working in areas where there was lead-dust on the floor and elevated surfaces, peeling lead-containing paint from the ceiling and walls, and fallen paint chips on the floor that were composed of 29.7%. lead.  Employees had been informed of the presence of lead, but they had not been provided with all of the comprehensive information in Appendices A and B of 29 CFR 1910.1025.
