29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On 20 and 21 June 2017, two workers were performing masonry work in which there was overhand laying of stone ongoing while on a scaffold without the use of head protection, exposing the two masons to the hazard of being struck by falling materials, tools, or debris.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces and support legs were used as a means of access. On 20 and 21 June 2017, two masons accessed a two tier tubular-welded frame scaffold by climbing the support legs of the scaffold, exposing both masons to a fall hazard of approximately 10 feet 3 inches and 14 feet to the ground below.
