OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to protruding, sharp-edged rotating parts of the braking system on a vault-lowering device that spun uncontrollably due to the drive shaft not being in place:    (a) Worksite:  On or about 6/1/2017, the employer did not ensure that the drive shaft component of a vault-lowering device (Wilbert Way) was in place to provide a controlled drop of the vault into the grave site.
