29 CFR 1910.269(m)(3)(ii): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be denenergized were open.  The employer did not render such means inoperable, unless its design does not so permit, and then sure that such means are tagged to indicate that employees are at work:  a) On or about 05/18/17 at the PeroxyChem substation located at 78 Sawyer Avenue, Tonawanda, New York; employees were performing preventative maintenance on the breakers in the substation. While preparing to work on breaker #6, an employee contacted live equipment and suffered burns and fractures. Switch #10 was left closed causing a backfeed from breaker #2 to breaker #6.  NO ABATEMENT CERTIFICATION REQUIRED
