Sharpsville Container Corporation, Production areas-EPG line area, On or about June 21st, 2017 and times prior thereto: Some employees voluntarily wore air purifying, half mask respirators with cartridges (3M brand #6200 respirator with Organic vapor cartridges) during work tasks including, but not limited to, grinding operations of metal. One employee was not provided with a medical evaluation prior to the voluntary use of the 3M brand half mask respirators. b.) Sharpsville Container Corporation, Production areas-EPG line area, On or about June 21st, 2017 and times prior thereto: Some employees were required to utilize supplied breathing air from a non-oil lubricated air compressor (Allegro compressor-model A1500TE; serial# 277257) through a supplied air respirator (Allegro series #9904 hood/helmet) during work operations inside of tanks onsite. Some employees were not provided with a medical evaluation prior to the required use of the supplied air respirators.

Sharpsville Container Corporation, Next to wash tank in the CPG area of the facility, On or about June 21st, 2017 and times prior thereto: Employees worked with and around corrosive chemicals including, but not limited to, Basic 1445L (potassium hydroxide). The emergency eyewash (Non-Pressurized Self-Contained Eyewash) in the chemical storage area (wash tank) was not in compliance with ANSI/ISEA 358.1-2014 for Emergency Eyewash and Shower Equipment in that: 1. Weekly visual inspections of the eyewash station were not conducted. The emergency eyewash unit was not activated weekly to verify proper operation.
