29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about 16 June 2016 at times prior thereto, an employee was performing roofing activities on the roof of an apartment complex clubhouse under construction while harnessed to a lanyard longer than the distance from roof to ground, allowing the employee to fall approximately 30 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about 16 June 2017 and at times prior thereto, the employer did not provide a training program to an employee assigned to work on the roof of an apartment building under construction, exposing the employee to the hazard of falling approximately 34 feet to the ground below.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On or about 17 June 2017, an employee received injuries after a fall that required in-patient hospitalization for treatment that was not reported by the employer within 24 hours.
