29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about 19 June 2017 and at times prior thereto, an employee without eye protection was using a pneumatic nail gun to install cornice on a multi-family residence under construction, exposing the employee to the hazard of being struck by flying debris.

29 CFR 1926.453(b)(2)(iv): Employee working in an aerial lift was not standing firmly on the floor of the basket.   On or about 19 June 2017 and at times prior thereto, an employee was installing cornice on a multi-family residence under construction while standing on the mid rail of an aerial lift, exposing the employee to the hazard of falling from the mid-rail and striking the basket.
