29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Location:  106 Old Waterbury Turnpike, Southbury, CT 06488  A Werner 20 foot fiberglass extension ladder utilized by employees to gain access to a roof did not extend 3 feet beyond the edge of a flat roof.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Location:  106 Old Waterbury Turnpike, Southbury, CT 06488  Each employee using extension ladders to access elevated walking/working surfaces (roof) were provided with a training program to recognize hazards related to ladders and procedures to be followed to minimize and correct hazards.
