29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  Jobsite: carpenters bracket scaffolding   Carpenters bracket scaffolding at the front and side of the house were not fully planked.  Employees on the scaffolding at the front of the house were exposed to a fall hazard of up to approximately 17 feet.

29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end.  Jobsite:  Carpenters bracket scaffolding at front of building, north side.  A scaffold plank extended over its support by approximately five feet.  No provisions had been made to secure the plank and prevent the cantilevered portion from falling if an employee stepped on it.

DeMelo dba De Melo Contracting ) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503 (a)(1), which was contained in OSHA inspection number 998011 citation number 1, item number 2 and was affirmed as a final order on November 4, 2017, with respect to a workplace located at 244 N.
