Employees in the shop using angle grinders with cutting wheels to cut chain links were exposed to struck-by hazards in that a hazard assessment to determine what PPE (Personal Protective Equipment) should be used to protect the employees during this operation had not been conducted.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  Employees engaged in operating a CAT forklift (#5CR4161), and employees near the forklift in operation, throughout the facility were exposed to struck-by hazards. The employees were exposed in that the operators were not provided comprehensive powered industrial truck training prior to operation.

29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months:   Employees in the west shop area using the 2 ton capacity overhead crane were exposed to struck-by hazards in that the crane was not inspected periodically.  The crane was found to have an electrical malfunction which caused the crane to be intermittently and unexpectedly inoperable while moving material.

The employees were exposed in that moving loads would suddenly stop due to electrical failure.  The employee(s) would jog/push the load manually.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4).  Employees engaged in grinding activities in the shop using the Bosh (Model 1811PS (3601H32A10) and DeWalt (SN 658712) hand held angle grinders were exposed to struck-by hazards in that the safety guards  were removed.
