29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    A.) Thomas Fosco dba Tom Fosco Roofing, roof of the structure, on or about June 27, 2017 and times prior thereto:  Employees were performing roofing activities that included, but were not limited to, removing shingles.  Employees were not utilizing a means or method of fall protection while exposed to a fall hazard of 11.7-feet to 19.7-feet.

Employees were exposed to a fall hazard of 11.7-feet to 19.7-feet.

Thomas Fosco dba Tom Fosco Roofing, on or about June 27, 2017 and times prior thereto:  The company had a damaged flexible cord on-site.  Damage included, but was not limited to, fraying of the outer jacket and fraying of the outer jacket at the cord cap.  The cord was not tagged out of service, rendered inoperable, or physically removed from the site.

Thomas Fosco dba Tom Fosco Roofing, on or about June 27, 2017 and times prior thereto:  Employees were accessing the roof of the structure with metal extension ladders.  Employees did not have ladder safety training.
