Thomas Fosco dba Tom Fosco Roofing, roof of the structure, on or about June 27, 2017 and times prior thereto: Employees were performing roofing activities that included, but were not limited to, removing shingles. Employees were not utilizing a means or method of fall protection while exposed to a fall hazard of 11.7-feet to 19.7-feet.

Damage included, but was not limited to, fraying of the outer jacket and fraying of the outer jacket at the cord cap. The cord was not tagged out of service, rendered inoperable, or physically removed from the site.
