Worksite: The employer did not ensure that the circular hand-fed rip saw that was used by employees to rip/cut trim boards was guarded with a hood that completely enclosed the portion of the saw above the table.

Worksite: The employer did not ensure that the employees who were required to work from the baskets of the aerial lifts were trained to recognize the hazards associated with the type of scaffold being used.
