29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about June 27, 2017, an employee working in a trench approximately 10.5 feet deep was exposed to overhead struck-by hazards.  The employee was not wearing a hard hat/protective helmet.     Abatement certification is required for this item in accordance with 1903.19(c).

A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.    On or about June 27, 2017, an employee working in a trench approximately 10.5 feet deep was not provided with a safe means of access/egress.      Abatement certification required for this item in accordance with 1903.19(c).

Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
