The latches on the box which are designed to keep it closed were broken which exposed employees to contact with live electrical components.  Item 2: On or about June 14, 2017, in the Tank/Railcar Washing Area, the employer did not ensure that the panelboard adjacent to Bay 5/6, which houses controls to return the wash water to the system, was maintained in a condition which permits it to be effectively closed.  The box was ineffectively closed with a flexible wire which exposed employees to contact with live electrical components.
