One must open a valve in order to supply water to the eye wash prior to activating it.

29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed:  Item 1: On or about June 14, 2017, in Bay 1/2 of the Tank/Railcar Washing Area, the employer did not ensure that the opening through which the conductors (for the camera) entered the cut-out box was effectively closed to prevent damage to the insulation on the conductors and to prevent water, dust and other foreign matter from accumulating in it.  The aforementioned electrical configuration was located beneath the grating of the Bay 1/2 work platform such that the metal cut-out box and/or the associated conduit was touching the metal grating.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures:  Item 1: On or about June 14, 2017, in the Tank/Railcar Washing Area, the employer did not ensure that the panelboard adjacent to Bay 3/4, which houses controls for the valves in the washing bays, was maintained in a condition which permits it to be closed.  The latches on the box which are designed to keep it closed were broken which exposed employees to contact with live electrical components.  Item 2: On or about June 14, 2017, in the Tank/Railcar Washing Area, the employer did not ensure that the panelboard adjacent to Bay 5/6, which houses controls to return the wash water to the system, was maintained in a condition which permits it to be effectively closed.  The box was ineffectively closed with a flexible wire which exposed employees to contact with live electrical components.
