29 CFR 1910.22(a)(3): (3) Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.    On or about 06/06/2017, in the kitchen/dishwashing area, at 400 N. Euclid, St Louis, MO, employees engaged in cooking operations were exposed to tripping hazards from floor tiles that are broken/cracked.

29 CFR 1910.37(a)(3): Exit routes were not free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.    On or about 06/06/2016, at the northwest corner of the building at the server workstation, milk crates and pizza boxes were stored in front of exit door.
