1910.178(l):  This standard is applicable to construction employment as defined by 29 CFR 1926.602(d).  Operators were not trained in the safe operation of powered industrial trucks:     The employer is failing to train employees on how to operate the powered industrial trucks and recognize the struck by or caught under hazards associated with their operation.  This most recently occurred on June 26, 2017 on the jobsite located at 1210 N 101st St, Lincoln, NE 68527.  The employer has inadequately trained employees operating and maintaining the forklift during residential home framing operations.  Instances of inadequate training include but are not limited to:  i. Employees operating powered industrial trucks without having received instruction on the safe handling operations as outlined in the operators manual and confirmed with a written test.  ii. Employees operating powered industrial trucks without having received practical training (demonstrations by the trainer) and practical exercises (performed by the trainee) on the safe handling operations as outlined in the operators manual.  iii. Employees operating powered industrial trucks without having been evaluated on their performance in the workplace.

1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    The employer is failing to ensure that all employees exposed to eye and face hazards from flying particles while working on a residential roof.  This most recently occurred on June 26, 2017 on the jobsite located at 1210 N 101st St, Lincoln, NE 68527.  One of the two employees was observed using an electric circular saw to frame a house without any form of eye protection.

This most recently occurred on June 26, 2017 on the jobsite located at 1210 N 101st St, Lincoln, NE 68527.  Employees were observed framing a ranch style home with a walk-out basement.  Roof is has an 8 in 12 steep sloped roof at heights of approximately 10 feet above the ground in the front and approximately 20 feet above the ground in the back.

This most recently occurred on June 26, 2017 on the jobsite located at 1210 N 101st St, Lincoln, NE 68527.  Employees were observed framing a residential home on an 8 in 12 steep sloped roof at heights greater than 6 feet above the ground below without utilizing fall protection.
