29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Location:  5 Country Road, Westport, CT 06880    Each employee exposed to fall hazards of approximately 15 feet above the ground while painting on a 6 in 12 pitched residential roof was not trained in fall hazards and the procedures to be followed to minimize such hazards.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  Location: 5 Country Road, Westport, CT 06880    An aluminum extension ladder used to access soffits for painting was not set up at the correct four to one angle.  On or about June 22, 2017, an employee using an aluminum extension ladder to access a porch to paint soffits fell off the ladder approximately 12 feet to the ground resulting in serious bodily injury.

Location: 5 Country Road, Westport, CT 06880    Each employee using extension ladders was not provided a training program to recognize hazards related to ladders and train each employee in the procedures to be followed to minimize these hazards.    On or about June 22, 2017, an employee using an aluminum extension ladder to access a porch roof to paint the soffits fell approximately 12 feet off a ladder to the ground and sustained serious bodily injury.
