The employer did not ensure that employees who were using a positioning device system kept their positioning line/s short enough to ensure the employee/s could not fall more than two feet. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards. The employer did not ensure that employees were adequately trained to recognize the fall hazards they were exposed to and the appropriate procedures to protect themselves from falling.
