29 CFR 1926.502(e)(1): Positioning devices were not rigged in such a way that an employee could not free fall more than 2 feet (.9 m).  The employer did not ensure that employees who were using a positioning device system kept their positioning line/s short enough to ensure the employee/s could not fall more than two feet. Employees were working on a roof with a pitch of 5 in 12 and were exposed to fall hazards of up to 27 feet eave to ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  The employer did not ensure that employees were adequately trained to recognize the fall hazards they were exposed to and the appropriate procedures to protect themselves from falling.
