Location - 175 School Street, Somerville, MA.  On or about June 30, 2017, eye protection was not used when an employee was using a pneumatic nail gun at a height greater than twenty four feet above lower level.

On or about June 29, 2017, an employee who fell was not protected from falling while working/walking on a roof at a height greater than twenty four feet above lower level.     (b) Location - 175 School Street, Somerville, MA.  On or about June 30, 2017, an employee was working/walking on a roof at a height greater than twenty four feet above lower level without fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) Location - 175 School Street, Somerville, MA.  On or about June 29, 2017, the employer did not provide training for the employees who might be exposed to fall hazard. An employee with no fall protection fell off the roof he was working/walking on.     (b) Location - 175 School Street, Somerville, MA.  On or about June 30, 2017, the employer did not provide training for the employees who might be exposed to fall hazard. An employee with no fall protection system in place to prevent him from falling was working on the same roof section that an employee fell off the day before (June 29, 2017).
