29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about June 27, 2017, the employer failed to ensure that the spool for the recycled vinyl being fed into the milling machine on line C1 was guarded to prevent it from being pulled off its stand.  An employee was cutting the roll when it was coming to an end, and the roll pulled off of its stand and crushed the employee's finger between the metal bar holding the roll and the bottom tray on the mixing  machine.

On or about July 6, 2017 the employer did not ensure that the tripwire cable on the mill mixer in area C1 was no more than 72 inches above the level where the operator is working.  The trip wire where the employee was working in area C1 was approximately 84 inches above the level where the operator stands.
