29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. At the construction site of a new multi-use building, the employer failed to provide appropriate training and correct ladder use. Employees leaned a 12 foot step ladder against 17 foot 9 inch wall and then used the top step to disconnect chains from the top of the wall exposing them to fall hazards of 12 feet to the concrete below.
