29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On or about July 6, 2017, at a worksite located at a new Ryan Home Development, Medina, OH 44256, during residential framing including setting walls on the second floor, employees started using the top and top step of a step ladder to install the top plate and were not trained in the construction, use, placement, and care in handling of all stairways and ladders.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    On or about July 6, 2017, at a worksite located at a new Ryan Home Development, Medina, OH 44256, during residential framing including setting walls on the second floor, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn while operating pneumatic nail guns at approximately 120 psi.   HY Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1199335, citation number 01, item number 001B, and was affirmed as a final order on February 6, 2017, with respect to a workplace located at 4707 Lima Trail, Medina, OH 44256.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)   On or about July 6, 2017, during residential framing activities, the employer did not ensure employees were protected from fall hazards of approximately 13.6 feet by ensuring a fall protection system/device was being used.     HY Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1199335, citation number 01, item number 003, and was affirmed as a final order on February 6, 2017, with respect to a workplace located at 4707 Lima Trail, Medina, OH 44256.
