29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:  On or about July 5, 2017:  a) An employee was exposed to a fall hazard of approximately 10.2 feet while utilizing a scaffold plank that was laid on top of another plank where it extended beyond the support on one end, and material on the porch roof at the other end.  The planks were supported by the horizontal components of the end frames.  b) Employees were exposed to a fall hazard of up to 17.4 feet while utilizing a red scaffold that was erected with two of its legs resting on scaffold plank that was resting on the horizontal components of the lower scaffold end frame and the other two legs were supported by the porch roof.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    On or about July 5, 2017:  a) Employees were exposed to fall hazards of approximately 10.2 feet when the employer failed to install a ladder on the scaffold they were using to gain access to the working area.
