29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) For employees working at 6120 Trevesta Place in Palmetto, as observed on or about July 6, 2017. Employees were observed working at heights greater than 6 feet without the use of a fall protection system. Employees were working at a height of approximately 25 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) For employees working at 6120 Trevesta Place in Palmetto, as observed on or about July 6, 2017. Employees were observed utilizing a step ladder to gain access to the upper landing surface that did not have side rails extended a minimum of 3 feet above the upper landing surface. Employees were exposed to a fall of approximately 25 feet.

Employees were observed utilizing a step ladder to gain access to the upper landing surface that was closed and leaned up against the upper landing area fascia board. Employees were exposed to a fall of approximately 25 feet.

Employees were observed working at heights greater than 6 feet without the use of a fall protection system. Employees were not trained on fall protection hazards.
