On or about July 7, 2017, at 107 Narrow Way Drive, Lafayette, LA the employer failed to ensure that employee(s) wore protective eyewear while using a nail gun to install roofing shingles.

Employees were installing roofing shingles to a residential structure.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July 7, 2017, at 107 Narrow Way Drive, Lafayette, LA the employer did not ensure that the ladder used for access  and egress to/from the working level of the residence were extended at least 3 feet above the landing.
