(a) Location - 10 Cottage Avenue, Arlington, MA.  On or about July 10, 2017, employees were not protected from falling while working/walking on a roof greater than twenty three feet above lower level.

29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall.    (a) Location - 10 Cottage Avenue, Arlington, MA.  On or about July 10, 2017, an employee was exposed to falling while ascending a WERNER extension ladder carrying a role of bulky roofing material which could cause the employee to lose his balance and fall.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.    (a) Location - 10 Cottage Ave, Arlington, MA 02474.  On or about July 7, 2017, employees' were not adequately instructed in the recognition and avoidance of unsafe conditions while working/walking on a roof without fall protection.      The PR Contracting Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.21(b)(21), which was contained in OSHA inspection number 898129, citation number 1, item number 1/a and was affirmed as a final order on 5/14/2013, with respect to a workplace located at   7 Monmouth Street, Lawrence, MA.
