29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) 343 King Street - The employer did not ensure that the manufacturer of the Toyota fork lift truck Model# BFUCU32 Serial# 12271 had given written approval for a drum clamp to be used with the truck on or before June 27, 2017.  Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use:  a) 343 King street - The employer did not ensure that the door leading to the interior motor control room was marked "Not an Exit", thereby exposing employees to improper egress routes on or before June 27, 2017.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
