29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Location - Otis and Winthrop Streets, Everett, MA:  On or about 7/11/17, an egress ladder in a trench did not extend three (3) feet above the street.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).   (a) Location - Otis and Winthrop Streets, Everett, MA:  On or about 7/11/17, an employee was working in an 8-foot deep trench without a protective system.    The GTA Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1) which was contained in OSHA inspection number 1104114, citation number 1, item number 3b, and was affirmed as a final order on 12/15/2015 with respect to a workplace located at 104 Brooks Street, East Boston, MA.
