29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a)  At the facility: The employer did not establish a Lockout/Tagout Program consisting of energy control procedures, employee training and periodic inspections for the maintenance and servicing of equipment including but not limited to the Mark Andy model numbers 2200 and 820 presses, as observed on or about July 12, 2017. 29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information:   a) At the facility: Employees worked with hazardous chemicals, including, but not limited to, chemicals such as UV Label Cure 354C and Clorox Bleach, and the employer did not provide these employees with the required training on the new label elements and safety data sheet format as required by the revised OSHA Hazard Communication Standard incorporating the Globally Harmonized System of Classification and Labeling of Chemicals prior to the effective date of December 1, 2013, as observed on or about July 12, 2017.
