29 CFR 1910.178(l)(1)(i): The employer had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (l) [Reference 29 CFR 1926.602(d) for construction]:    Employee(s) engaged in the installation of siding on the east side of the building that was located at 11691 South   Ridgeview Road in Olathe, Kansas were exposed to struck by / crushing hazards in that employees were not instructed in the recognition of hazards associated with working on a powered industrial lift to nail siding to the building.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    (a)  Employee(s) engaged in using DeWalt air compressor to power pneumatic nail guns on the east side of the building located at 11691 South Ridgeview Road, Olathe, Kansas were exposed to electrical hazards due to the power cord being damaged.  The outer insulation had separated from the plug end approximately 3 inches, thus exposing the energized conductors.    (b)  Employee(s) engaged in using a green extension cord to power a portable air compressor on the north side of the building located at 11691 South Ridgeview Road, Olathe, Kansas were exposed to electrical hazards due to the extension cord being damaged.  The outer insulation had separated from the plug end approximately 1/2 inch, thus exposoing the energized conductors.
